REGISTRATION INSTRUCTIONS
1. Fill out the Medical Release form.  BE SURE TO GET IT NOTARIZED! This can be done at most banks, government, and  legal offices
2. Sign the “Do’s and Don’ts” form.
3. Include the extra-curricular participation fee: $40 (checks made out to Westwood High School).  Family cap is $300 for all extra-curricular activities (sports and clubs/organizations).
